Single Choice fields in Basic Forms

This article explains how to add and configure Single Choice fields in the Basic Form Builder.

Admins and Designers can use Single Choice fields to ensure users pick one option, improve data accuracy, and standardize values for reporting.


Single Choice fields

A Single Choice field is best when users must pick one and only one option from a short, predefined list.

Please Note

Reference Data cannot be used with this field type.

Add a Single Choice field

To add a new Single Choice field in the Basic Form Builder:

  1. Create a new form using your preferred Basic Form Builder option.

  2. Add or select a Single Choice field using the applicable method:

    • If a field already exists on the PDF: Select the field on your PDF. On the right side, in the Field panel, expand the Type dropdown menu and select Single Choice.
    •  If adding a new field: On the left side, in the Add panel, drag a Single Choice field onto your PDF.

  3. On the right side, scroll down to the Choices panel.
  4. In the Choices panel, select Start from scratch or Edit a common list:

    • Start from scratch to add custom options.
    • Edit a common list to use the following predefined editable options:
      • US State Abbreviations 
      • US State Names 
      • Month 
      • Weekday 
      • Australian State Abbreviations 
      • Australian State Names 
      • Canadian Province Abbreviations 
      • Canadian Province Names 
      • Pass/Fail

    Note

    The Clear All option is also available in the Choices panel to quick remove all choices at once.

  5. Select a value from the Default Value dropdown menu to pre-fill when users start a new submission. Consists of the following options:
    • Blank 
    • First Choice 
    • All options you added to the Choices panel, either custom or from a common list
  6. In the upper-right corner of the Builder, click Save, then Test the field, and click Publish and Assign to users.

Note

The Choices panel list will be saved if you switch between Single Choice Boxes or Radio Style.

Configure a Single Choice field

The Style and Required settings for the Single Choice field control how choices look and behave when users fill out a form.

Style

Controls how the Single Choice field looks and behaves for users filling out the form. The options include:

  • Radio: Displays all available options in a vertical list next to round selection buttons.

  • Boxes: Displays all available options (from two to five choices) in a single row of tappable boxes.

Note

Boxes have limited character space so keep Choices short and readable to prevent them getting cut off, as demonstrated in the screenshot above.

Required

Makes the Single Choice field mandatory. Users must enter a value before moving to the next screen or submitting the form.

Single Choice field values on PDFs

The Standard PDF and Quick Start PDF will not display the full list of Choices. Only the Choice selected in the submission will be shown as plain text.

When editing submission data in the web portal, the list of predefined Choices does not appear and the field value must be updated manually. Alternatively, the user can edit the submission on mobile or Edit in Web Client from the Submissions page.
